Social Work & Key Worker Model Service
Social Work & Key Worker Model Service
Our social workers are dedicated to empowering individuals to achieve their life goals, providing safety and stability during times of hardship or vulnerability.
We offer unwavering support to participants and their families as they navigate the NDIS, tailoring our assistance to fit their unique needs. Our team not only possesses extensive knowledge in the NDIS sector, but they also have significant experience in mainstream and community services. We are committed to offering our services in person, by telephone, in the community, at our office, or even in your home to provide maximum flexibility and support.
Here are some ways your social worker can offer help:
At Skye’s The Limit, our social workers possess a wide-ranging skill set and are ready to assist in various ways, including:
- Guiding you through navigating mainstream services and NDIS services, if required.
- Conducting comprehensive assessments (e.g. living skills, behaviour, environment, social abilities, etc.).
- Strengthening the skills and capacity of family members.
- Fostering increased independence for individuals.
- Supporting families to maintain their caring arrangements (e.g. through respite, personal care support, or domestic assistance).
- Support to comprehend your service plan and how to access and prioritise resources to achieve its goals.
- Offering support and counselling directly or connecting you with the appropriate services.
- Providing vital information about government benefits and other community resources.
- Linking you to support groups or other families.
- Developing strategies to assist with behaviours of concern or mental health issues.
- Providing information and education about relationships, friendships, and sexuality.
Additionally, your social worker can extend support and connect you to appropriate expert help during tough situations such as:
- Coping with the loss of a family member or friend.
- Managing financial difficulties.
- Overcoming post-traumatic events.
- Dealing with domestic violence.
- Navigating through grief and loss.
- Handling the challenges of parenting.
A Key Worker is an early childhood intervention professional, and at Skye’s The Limit, our social workers can harness their experience and skills to serve as Key Workers.
Your Key Worker becomes the primary point of contact for all your family’s needs and is the person you’ll interact with the most.
Our Key Workers embrace a family-centered approach – we recognise the significance of working in partnership with families. By collaborating with families, we strive to provide them with choices and control over how to support their child.
What does Key Worker do?
Similar role as a Social Worker, Key worker is offering:
- Support as you meet with a NDIS planner.
- Providing guidance to help you make informed decisions about your child’s development.
- Assisting in setting goals for your child that align with your priorities.
- Supporting the integration of learning goals into your daily activities and routines.
- Assisting in accessing local community services and activities, such as childcare, school, shopping centers, local parks, or sports groups.
